In operation since 1985, Hope Hospice and Palliative Care, Inc. of Rib Lake, Wisconsin, is an independent, non-profit, tax exempt corporation which provides hospice and palliative services to persons in all of Taylor County, Price County south of Highway 8, Clark County north of Hwy. 29 and portions of Lincoln, Rusk and Marathon counties. Mission of Hope Hospice and Palliative Care

Hello and welcome to our website.

Hope Hospice and Palliative Care has been helping people in our community since 1985. Our mission is to provide care for persons at the end of life so they may live as comfortably as possible and to offer continuing support to the bereaved.

Hope Hospice offers sensitive care and understandingHospice seeks not to add days to one's life, but rather add life to one's days. Hospice strives to offer understanding and sensitive care in a supportive setting. Hospice care neither hastens nor postpones death, but works to enable patients and families to attain a degree of emotional preparation for death. This preparation can bring a sense of serenity and fulfillment.

The care provided is tailored to fit the individual needs of each patient and family. Hospice works directly with the patient's own physician to provide optimal care and symptom management, while maintaining maximum patient alertness. A medically supervised interdisciplinary team of professionals and volunteers coordinates the services.

The patient may receive hospice care either in his or her own home or a senior living facility. Services provided can include a number of things, such as professional nursing care, serving as patient advocate in obtaining information from physicians or other resources and assisting with financial matters, wills, funeral planning of other such tasks.

Patients and their families often prefer to have care provided in the person's own home as long as possible. Support services can extend that period of home care. Hospice workers can coordinate and consult with other community services and groups that might be able to help provide a maximum of patient and family support. Coordination can be arranged with other agencies for needed services, such as Meals on Wheels or Care Call.

The workers can assist with household chores and errands. Many times the listening ear and understanding heart provided by a hospice worker can be an invaluable aid. Providing companionship and relieving loneliness can be a very important part of a hospice worker's effort.

Through hospice programs, the patient, family, and other loved ones can gain assistance in understanding and working through the grieving process. Hospice can continue to provide bereavement care to the family for one year after the patient's death or for as long as needed.

When life is limited, it becomes more precious. The hospice staff assists family members to communicate honestly and openly with each other, often rendering those last weeks the most satisfying and love-filled weeks ever experienced by the family.

We also offer a Grief and Loss Program that provides services to grieving families.

Referrals to Hope Hospice

Referrals from family, friends, neighbors or doctors.

Referrals come to Hope Hospice from a number of sources. Physicians and hospital discharge planners will recommend our services to patients and their families who they feel will benefit from our services. But we don't require a medical referral. Anyone can make a referral. Neighbors, friends and family members who are aware of the needs of their loved one may contact us directly.

Funding

Hospice care is based on need, not on ability to pay. Medicare, Medicaid and many private insurance plans include hospice benefits which reimburse part of the expenses.
