The patient and family are the central part of the hospice care team. Other members of the team include the patient's physician, the hospice director, medical director, registered nurses, social workers, home health aides, chaplain, and trained volunteers. Physical, occupational, speech, and diet therapy are also available through Hope Hospice and Palliative Care.
