MEDICAL SOCIAL WORKER

We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will be responsible for providing support and resources to terminally ill patients and their families. This role requires a strong understanding of social work principles, particularly in clinical and hospice settings, and a commitment to improving the well-being of individuals facing medical challenges and end of life.

Duties

• Conduct comprehensive assessments of patients’ psychosocial needs and develop individualized care plans.
• Provide crisis intervention and support to patients and families during difficult times.
• Collaborate with healthcare professionals to ensure holistic care for patients, including those in geriatrics and pediatrics.
• Assist patients with dementia care strategies and provide education to families on managing related challenges.
• Facilitate access to community resources, including financial assistance, housing, and mental health services.
• Document patient interactions and progress in compliance with hospice policies and regulations.
• Participate in interdisciplinary team meetings to discuss patient care strategies and outcomes.

Requirements

• Valid state licensure as a Clinical Social Worker (LCSW), MSW, or CSW.
• Experience working hospice or medical field is preferred.
• Familiarity with geriatric care practices, as well as experience working with children is a plus.
• Strong communication skills, both verbal and written, with the ability to empathize with patients from diverse backgrounds.
• Ability to work collaboratively within a multidisciplinary team environment.
• Knowledge of community resources available for patients requiring additional support.
• Requires traveling within a 100 mile radius which requires reliable transportation and a valid driver’s license.
• This position is not remote.
