Hope Hospice and Palliative Care, Inc is seeking a full time Social Worker to join our team! A Bachelor’s Degree in Social Work from a school of social work accredited by the Council on Social Work Education is preferred; along with a possession of a certificate or license and current registration to practice social work in the State of Wisconsin.
This candidate must be familiar with the hospice concept and its implementation. They must have the ability to identify and assist in coordinating the needs of the terminally ill and their families. They should have the ability to understand and promote effective interaction between the members of the hospice team, nursing homes, hospitals, and community members. Daily travel is common. Candidate Must be willing and able to travel within the hospice service area (90-mile radius- all of Taylor County and portions of the surrounding counties).
The candidate must be able to perform management tasks and administrative functions as directed by the Manager of Social Services and/or by the Executive Director.
This position will be responsible for coordinating with Hospice Bereavement & Volunteer services and the following to patients and families: psychosocial assessment, crisis intervention, assistance with social, economic, and survival needs, advocacy, information and referral, case management, completing advance directives and other requests made by patients or families.
