The care provided is tailored to fit the individual needs of each patient and family. Hospice works directly with the patient’s own physician to provide optimal care and symptom management, while maintaining maximum patient alertness. A medically supervised interdisciplinary team of professionals and volunteers coordinate the services.
The patient may receive hospice care either in his or her own home or a living facility. Patients and their families often prefer to receive care in their own home for as long as possible. Support services can extend that time period in the home.
