Our hospice social worker is an exceptional resource who can provide assistance with practical and financial concerns as well as emotional support and bereavement care follow-up.
Our social worker evaluates the need for other support services by the family and facilitates communication between the family, community agencies and other needed services.
They can assist the patient and family with Advance Care Planning. Advance Care Planning is making decisions about the care you would want to receive if you become unable to speak for yourself.
